00:36Welcome to Aao Milkar Banai Pakistan
00:38An initiative of ARYB Global
00:40Sehatman Pakistan
00:42Affordable and accessible
00:44health for everyone
00:46Today we have with us
00:48a prominent doctor
00:50Prof. Dr. Ashar Fawad
00:52He is a more than 25 years
00:54experienced doctor, researcher
00:56and consultant
00:58He is a medical biologist. He is serving as a Dean, Director Research and Joint Secretary in BIDE and DAP.
01:08In Pakistan, he is the founder of the Diabetes Registry and the head of the National Diabetes Survey.
01:14He is providing free insulin to more than 3,500 diabetes patients through IML.
01:24He is the author of more than 180 research papers and is associated with many international and national institutions.
01:33Dr. Ashar Fawad, welcome back.
01:38Thank you very much for your kind introduction and providing this platform once again.
01:43Sehatman Pakistan, always with you, Inshallah.
01:48First of all, I have a question for you.
01:51I have some more questions for you and you need to answer them quickly.
01:55Please tell me, what are the causes of obesity?
01:58Let me speak in Urdu so that the public can understand.
02:03If I speak quickly, obesity is caused by genes and environment.
02:09We have inherited genes from our forefathers.
02:13Now the question is, there was no obesity in our forefathers.
02:17Their lifestyle and environment interaction was different.
02:22They did not have a machine life.
02:24They walked a lot.
02:26Their physical activity and calories burning was involved in all their activities.
02:30In our environment, the change happened that all these things were completely reversed.
02:36These are the main causes of obesity, change in environment.
02:40If genes are present and the environment changes,
02:45but there is no interaction, then a person will not be obese.
02:49Even in today's time, people who maintain their lifestyle are not able to become obese.
02:54Absolutely right.
02:55And link of other diseases with obesity?
02:58If you say that obesity affects everything from head to toe,
03:07then it is not wrong at all.
03:10If I talk about non-communicable diseases,
03:13which are heart diseases, diabetes, cancer, psychological disorders, joint pains,
03:19you name anything.
03:21After joint pains, there is arthritis.
03:23If someone has that disease, if he is obese, then he will be more affected.
03:33Like we went through Covid, obesity was a risk factor.
03:36This is a simple example.
03:38Similarly, if there is a heart patient, he will be obese.
03:41The third thing is that what do we understand about obesity?
03:45This is important.
03:46Absolutely right.
03:47What can be helpful from your routine and diet to decrease obesity or get rid of it?
03:55The first thing is to understand obesity.
03:58We have this concept that if we look healthy, then it is good.
04:05But it is not like that.
04:07It is good, it can be towards obesity.
04:10Whether it is a child, whether it is of middle age, whether it is of old age.
04:14Guidelines and cut-offs have been made all over the world.
04:17What is your waist, hip circumference, BMI?
04:21We can judge this and adjust it with diet and lifestyle modification.
04:27But there are responders.
04:28If we look at 50 people, not everyone will respond to the same diet.
04:33We need to judge it first.
04:35And then we have to prescribe individualized diet plan and lifestyle modification plan for the responders.
04:43Absolutely right.
04:44Now tell me, how helpful is a walk?
04:48On a regular basis, please motivate for a walk.
04:52Because the interest and routine for a walk has become less in our life.
05:00There is another caller.
05:02Let's take a call first.
05:03Absolutely right.
05:04Salam.
05:05Salam Salman bhai, how are you?
05:07I am fine, Alhamdulillah.
05:08Dr. Fawad is with us.
05:10What is the question brother?
05:11My question was about diabetes and cancer.
05:16First, I wanted to know that, God forbid, if someone has this cancer, how can it be diagnosed?
05:24What are its symptoms?
05:25What kind of test can we do to diagnose it?
05:29And secondly, there is diabetes.
05:30How can we diagnose it?
05:32I mean, what are the symptoms?
05:34We have to feel it.
05:35Then we go to the laboratory and do our tests.
05:37And then its precautions come later.
05:39But what is the proper way to diagnose it?
05:41Please guide me on this.
05:42Okay.
05:43This is a very good question.
05:45See, to diagnose diabetes, as I told you in the last program,
05:49you can do a simple fasting of 8 to 10 hours.
05:52Fasting plasma glucose level test can be done from any lab.
05:56If it is below 100, then it is normal.
05:58If it is between 100 to 125, then it is borderline.
06:02We call it impaired.
06:04And if it is above 125, i.e. 126 and onwards, then it is diabetes.
06:09And after that, there are more detailed tests.
06:12As far as cancer is concerned, my cancer presents in many different ways throughout the body.
06:19So, whenever you come to the doctor with any symptoms,
06:23first the primary care physician judges that these symptoms are normal,
06:29like it has become viral, fever and is going to be fine.
06:33Or it has been going on for a long time.
06:35So, if any of the symptoms have been going on for a long time,
06:38like weight loss, lack of appetite, or if there is cancer in the stomach,
06:45then there is pain there.
06:46So, it will be a very vague thing to close the river in the well,
06:50that the diagnosis of cancer is possible with a single test.
06:53These are the symptoms.
06:54And then on the basis of these symptoms, the tests are carried out.
06:58It has become completely fine.
06:59May I answer your question?
07:02The element of walk.
07:04Absolutely.
07:05If we talk about walk, then you will remember that some 20 years ago,
07:10if I take the name, then it is not a bad thing.
07:14There used to be an advertisement of Mr. Wasim Akram that you don't get tired.
07:18So, the thing was that one thing was smoke and the other thing was shown to him while walking.
07:23Absolutely right.
07:24So, walking and as I said in my first question that everything is on the machine.
07:29Now, a gentleman was saying somewhere that it is enough for us
07:33that we go to the counter of our shopping center.
07:37Absolutely.
07:38This has become a habit in us.
07:40So, you are right that we need to change this culture.
07:43The thing about walk is that it is basic.
07:47After that, it is intensified as per the individual requirement.
07:51If you don't see any benefit, you are not losing weight,
07:55but the walk that is giving you internal benefit of metabolism,
07:59you can't judge it.
08:02So, if you do 15 to 20 minutes of brisk walk, 100 steps per minute,
08:07if you are not a heart patient and otherwise fit,
08:10then that works like nothing.
08:12Absolutely right, doctor.
08:14I do agree with your point.
08:16Because I have traveled with Mr. Wasim Akram from Pakistan.
08:20And I have seen that 25 years ago, he is fighting with his diabetes also.
08:25But he has to run 8 kilometers early in the morning.
08:28And this is not easy for a diabetic patient.
08:32He has to run 8 kilometers, not walk.
08:35It is like jogging, kind of.
08:38So, after walking with him for some time,
08:40I realized that we have walked a lot.
08:43So, I asked him, how much will you walk?
08:45After hearing 8 kilometers, I went back.
08:47I said, you go, it will take time.
08:49So, if this person wants,
08:51even after becoming a diabetic patient,
08:53because he is on insulin,
08:55even after becoming a diabetic patient,
08:57he can keep himself super-duper fit.
08:59If he has the element of walking in his life,
09:01and as the doctor said,
09:03routine and diet, have some change.
09:05Because your health is the biggest wealth and blessing.
09:09If you don't have health, you are not physically fit,
09:12then whatever worldly rewards you have,
09:14wealth, there is no use of it.
09:17No one will give you any benefit.
09:19Our website is on the screen.

09:41And for the call center,
09:43dial 021-111-000-162
09:48and through this,
09:50you can join us.
09:52And anytime, to register us,
09:54you can understand the method and procedure.
09:57Let's include one more caller.
09:59Assalamualaikum.
10:01Assalamualaikum.
10:02Walaikumassalam.
10:03Yes, what is your question?
10:05My question is,
10:07I am a diabetic patient.
10:09Okay.
10:10And I have been diabetic for about 8 years.
10:12Okay.
10:13And the thing is,
10:14I can't walk,
10:16because the bones of my right leg,
10:18the bones of my right leg,
10:20they say,
10:21if I get a nose,
10:22then it is difficult for me to walk.
10:24Okay.
10:25I am a heart patient.
10:26You are a heart patient.
10:27And I am diabetic.
10:28Diabetic.
10:29What is your age?
10:30My age is about 60.
10:3260.
10:33Yes.
10:34Alright.
10:35Okay.
10:36Okay.
10:37Doctor, tell us.
10:38Yes,
10:39many people like you have this problem.
10:42Obviously, we call it comorbid.
10:44For example,
10:45arthritis,
10:46knee pain,
10:47can't walk,
10:48can't jog,
10:50can't cycle.
10:51So, we do have rehab plans
10:53and physiotherapy plans for them.
10:55In which,
10:56by doing upper body exercises,
10:58you can compensate for it.
11:00You will learn that exercise
11:02according to your need.
11:04And then,
11:05it will compensate for your walk.
11:08Again,
11:09you have to keep your things fit.
11:11That is,
11:12you have to control diabetes.
11:13The factors of your heart,
11:15the doctors are telling,
11:17blood pressure,
11:18cholesterol,
11:19each and everything should remain controlled.
11:21Then,
11:22you will be able to have,
11:24to be physically active.
11:25In our mind,
11:26the concept is that
11:27we are only taking walk,
11:29or walking,
11:30or running.
11:31Our,
11:32like we are talking right now,
11:34by sitting,
11:35compared to standing,
11:36you will burn more calories.
11:38By walking,
11:39you will burn more calories.
11:40By walking fast,
11:41you will burn more calories.
11:42By lying down,
11:43you will burn less calories.
11:44So,
11:45this concept should be clear,
11:46that you can get things from upper body exercises.
11:50Doctor,
11:51did you see this type of case,
11:53that genetic diabetes,
11:55or diabetic patients,
11:57genetically,
11:58definitely,
11:59you know,
12:00they move.
12:01But,
12:02did you ever see,
12:03that no,
12:04by taking precautions,
12:05you were saved from it.
12:06And,
12:07that thing didn't come in children.
12:08Is that possible?
12:09Yes,
12:10definitely.
12:11This is a very good question,
12:12which I wanted to ask from
12:13the platform of
12:14Sehat Ban Pakistan,
12:15on prevention.
12:16That,
12:17if you,
12:18see,
12:19genes,
12:20being Asians,
12:21many scientists,
12:22who have researched Asia,
12:23for 50-50 years,
12:24said that,
12:25being Pakistani,
12:26Indian,
12:27Bangladesh,
12:28Nepal,
12:29Sri Lanka,
12:30Bhutan,
12:31this is one risk factor,
12:33that you have.
12:34Meaning,
12:35you,
12:36while carrying genes,
12:37inside you,
12:38and,
12:39on that,
12:40preventing you,
12:41yes,
12:42it's possible.
12:43If you,
12:44maintain your good lifestyle,
12:45in lifestyle,
12:46both things come,
12:47diet,
12:48and activity,
12:49and then,
12:50early identification.
12:51People are afraid of medicines,
12:52but,
12:53if you,
12:54identify early,
12:55like,
12:56I talked about borderline,
12:57so,
12:58at that time,
12:59with medicine,
13:00diabetes proper,
13:01can be stopped from converting.
13:02After that,
13:03blood pressure,
13:04cholesterol,
13:05heart disease,
13:06and other important things,
13:07see,
13:08longevity of life is there,
13:09Allah has given us life,
13:10now,
13:11in life,
13:12there should be no complications,
13:13and our quality of life,
13:14should be good.
13:15So,
13:16yes,
13:17it's possible,
13:18if you,
13:19with your children,
13:20correct your lifestyle,
13:21from the beginning.
13:22What happens with us,
13:23there is one diabetic,
13:24so,
13:25he will be given,
13:26separate food.
13:27We,
13:28are educating,
13:29that the whole house,
13:30will have to change,
13:31the whole community,
13:32will have to change,
13:33and for that,
13:34this prevention is possible.
13:35Absolutely right.
13:36Thank you so much,
13:37Dr. Ashar Fawad,
13:38for coming here,
13:39and,
13:40in this short duration,
13:41you solved,
13:42a lot of,
13:43health issues,
13:44and,
13:45in terms of prevention care,
13:46see,
13:47you provided,
13:48such an amazing solution,
13:49that,
13:50we can save ourselves,
13:51from genetic diseases,
13:52we can save our lives,
13:53because,
13:54if we,
13:55go towards prevention,
13:56go towards preventive care,
13:57then definitely,
13:58we will live a healthy,
13:59wealthy life.
14:00And,
14:01for that,
14:02the initiative of,
14:03Healthy Pakistan,
